Now that you have had your intake discussion and have been assigned a therapist that best fits your needs and schedule… it is time to confirm your chart information!
Click on your therapist’s name and enter the username and password you created. Sign into your account to access your receipts, statements, and appointment information at any time. If you have not yet created a username and password to login, please email intake@northshorefamilyservices.com to resend an invitation link to create a login.
